1. First, you must submit a General Statement of Need.
2. Secondly, you must submit a Physician’s Assessment Form.
Both of these forms are available from ACRA. The application process seeks to identify what your needs are, and to determine if/how ACRA can meet those needs. We recommend that you speak to an ACRA staff member before beginning the application process.
